1.Title:
称呼,即称先生、小姐、女士、夫人还是某某博士
2.Familyname(SurnameorLastname):
姓
3.Givenname(FirstnameorChristianname):
名
4.Dateofbirth:
出生日期。
英国人习惯按日/月/年(dd/mm/yyyy)的顺序而美国人习惯按月/日/年(mm/dd/yyyy)的顺序
5.Homeaddress:
家庭地址。
注意顺序,从门牌号、街道名、城市名到国家名、邮政编码,从小到大依次填写。
6.Countryofbirth:
出生国,即在哪个国家出生。
7.Nationality:
国籍,即现在是哪个国家的公民。
注意:
此处应用国名的形容词形式,如例文中用到American而不是America或U.S.A..中国国籍用Chinese而不用China。

动词的时态:
一般现在时
1)经常性或习惯性的动作,常与表示频度的时间状语连用。
时间状语:
every…,sometimes,often,always…

2)客观真理,客观存在,科学事实。

3)格言或警句中。
Pridegoesbeforeafall. 骄者必败.
注意:
此用法如果出现在宾语从句中,即使主句是过去时,从句谓语也要用一般现在时。

一般过去时:
1)在确定的过去时间里所发生的动作或存在的状态。
时间状语:
yesterday,lastweek,anhourago,theotherday,in1982

2)表示在过去一段时间内,经常性或习惯性的动作。

3)句型

一般将来时
1) shall用于第一人称,常被will所代替。
will在陈述句中用于各人称,在争求意见时常用于第二人称。

3) be+不定式表将来,按计划或正式安排将发生的事。

注意:
beaboutto不能与tomorrow,nextweek等表示明确将来时的时间状语连用。
beto表示客观安排或受人指示而做某事。
begoingto表示主观的打算或计划。

一般现在时表将来
1)下列动词:
come,go,arrive,leave,start,begin,return的一般现在时表将来。
这主要用来表示在时间上已确定或安排好的事情。
Thetrainleavesatsixtomorrowmorning.
2)在时间或条件句中。
WhenBillcomes(不是willcome),askhimtowaitforme.
用现在进行时表示将来
意为:
“意图”、“打算”、“安排”、常用于人。
常用词为come,go,start,arrive,leave,stay等。
I'mleavingtomorrow.
现在完成时
现在完成时用来表示之前已发生或完成的动作或状态,其结果的确和现在有联系。
动作或状态发生在过去但它的影响现在还存在;也可表示持续到现在的动作或状态。
其构成:
have(has)+过去分词。
HehasbeenintheLeagueforthreeyears.(在团内的状态可延续)
HehasbeenaLeaguememberforthreeyears(是团员的状态可持续)
HejoinedtheLeaguethreeyearsago.三年前入团,joined为短暂行为
句子中如有过去时的时间副词(如yesterday,lastweek,in1960)时,不能使用现在完成时,要用过去时

用于现在完成时的句型
1)Itisthefirst/secondtime….that…结构中的从句部分,用现在完成时

2)Thisisthe…that…结构,that从句要用现在完成时.
