6.0-DataDrivenCorrectiveAction6.01NearMiss6.02FirstAid7.0InjuryClaims7.01ReturntoWork7.02ClaimsManagement3.0AuthorizationTheGeorgetownToolboxPlantisresponsiblefortheestablishment,implementation,andenforcementoftheIncident&InjuryManagementProgram.ThePlantManager,DepartmentSupervisors,andHRManagerwillensurethiscommitment.TheHRManageristheIncident&InjuryManagementChampionandisresponsibleformaintaininganddistributingthePlan,whereacopyofthePlanisavailableforreviewand/orcopyuponrequest.ThefollowingtitleshavebeenidentifiedandassignedspecificresponsibilitieswithinthePlan:
3.1FirstRespondersIndividualswhohavereceivedinstructioninthedangersofBloodBornePathogens(BBP)andtrainingintheapplicationofFirstAid.TheFirstRespondersareresponsibleforassessingtheseverityofanywork-relatedinjury/illness,providingon-sitetreatment,and/ordeterminingifexternaltreatmentisrequired.4.0CompanyPolicyInanefforttoensurethehealthandsafetyofemployees,theGeorgetownToolboxPlanthasmandatedthatallincidents,nomatterhowminor,suchasinjury,illness,discomfort,orevennearmisses(closecalls)aretobereportedtoaSupervisororHRManager.Reportingmustbemadebytheendoftheemployeesworkday,orwithin24hoursoftheoccurrence.5.0CompliancewithProgramAllemployeesarerequiredtocomplywiththerestrictions,limitations,procedures,andpoliciesestablishedbytheIncident&InjuryManagementProgramandanyrelatedcompanypolicies.Failuretocomplywillresultindisciplinaryaction.6.0Communication&TrainingTheGeorgetownToolboxPlantwillprovideappropriateinstructionstoallemployeestoensurethatthepurposeandfunctionoftheIncident&InjuryManagementProgramareunderstood.AllnewemployeeswillreceiveinitialtrainingaspartoftheirNewHireOrientation.AnnualrefreshertrainingwillbeprovidedforalltheGeorgetownToolboxPlantemployees,wheretheywillberequiredtosign-intodemonstratetheirunderstandingandcommitmenttoobeyingtherequirementslistedintheIncident&InjuryManagementProgram,especiallytheeligibilityrequirementsforWorkerCompensationandtheReturn-to-WorkProgram.Re-trainingwillbeconductedwheneveranewpolicyisintroduced,achangeismadetocurrentpolicies,programsorprocedures,orinadequaciesinemployeeknowledgeareidentified.6.1SafetyDialoguesInanefforttopreventanyEHSincidentsfromoccurring,GeorgetownToolboxPlantManagementholdsconversationsbetweenManagers/SupervisorsandGeorgetownToolboxPlantemployeeswhichfocusonEnvironmental,Health,andSafety(EHS)matters/topicsTheyareanimportanttooltoenhancingcommunicationsastheyprovideanopportunityforemployeestosharetheirEHSideasandconcernswiththeirleaders.SafetyDialoguesareconductedwithProductionsemployeemonthlyanddocumentedonatrackingspreadsheet.FindingsfromtheEHSDialoguesmaybedocumentedusingtheNearMissReport(SeeSection8.1.1).6.2EHSMeetingsTheHRManager,ordesignee,holdsmonthlyEHSmeetingsfortheProductionemployeeswhereadifferentEHSrelatedtopicisreviewedforgeneralawarenessand/ortrainingpurposes.Incidentandinjurydataispresentedtotheemployeeeachmonthatthebeginningthemeeting.RelevantinformationfromthePlanisalsodistributedtoaffectedexternalmedicalprovidersandthethirdpartyworkerscompensationinsurancecarrier.Thepartiesareissuedanannualinvitationtotourthefacilityinordertofamiliarsthemselveswiththeassociatedjobsfunctionsand/orincreasetheirunderstandingoftheoperationsorworkingconditions(SeeAppendixD)7.0ProgramReviewTheentireIncident&InjuryManagementProgramwillbereviewedonanannualbasisandupgradedwhennecessarybytheHRManager.ResultsoftheannualreviewwillbereportedtoGeorgetownToolboxPlantManagement.Ifnecessary,copiesofanychangeswillbesenttoaffectedexternalproviders/agencies.8.0ContentsofProgram8.1Equipment&SuppliesGeorgetownToolboxPlanthasanon-siteFirstAidRoom,whichhassufficientsuppliesandequipmenttohandleallminorinjuriesorillnesses,andtreatorstabilizeanymajorinjuriesbeforetransporttoanoutsidemedicalfacility.Thefacilityisequippedwithmultiplefirstaidkits,eyewashstations,emergencyshowerswhichareallstrategicallylocatedthroughoutthebuilding.8.2Reporting,Investigating&Documenting8.2.1NearMissNearmiss(closecall)reportingisapplicabletoanyincidentoreventthatoccursbutthatdoesnotresultinanemployeeinjury/illness.Nearmissesmayresultinpropertydamageorcausesunsafeworkingconditions.Allnearmissincidentsmustbereportedwithin24hoursofoccurrenceanddocumentedusingaNearMissReport.Nearmissincidentsareclassifiedasminor,serious,major,orcatastrophicbasedonthegravityofpotentialpersonalharm.Althoughcontributingandrootcausesshouldbeidentifiedforallnearmissincident(ontheNearMissReport)furtherinvestigationand/orevaluationisrequiredforallserious,major,orcatastrophicnearmisses.Thedetailsofallincidentinvestigations,includingthecorrectiveactions,withownersanddeadlinestoclosethefindings,mustbedocumented(within24hrs).Correctiveactionsshouldbeglobalacrossthesite,applyingtoallsimilarequipmentandallaffectedemployees.Supervisorsareresponsibleforcompletingandsubmittingassociateddocumentation,however,blankformsareavailabletoallemployeesinbothelectronicandhardcopyformat.CompletedformsmustbesubmittedtotheHRManager.8.2.2InjuryandIllnessAllwork-relatedinjuriesorillnesses,regardlessoftheirseverity,mustbeimmediatelyreportedsothateffectivetreatmentmaybeprovided.AnEMSmemberorSupervisormustbeallowedtoprovidetheinitialassessmentforallwork-relatedinjuries.Injuriesthataretreatedon-sitebyaFirstResponderorSupervisormustbedocumentedusingtheFirstAidReport.TheformmustbefilledoutbytheFirstResponderorSupervisoratthetimeoftreatmentandthenimmediatelysubmittedtotheHRManager.Injuriesthataretreatedoff-sitebyanexternalagency(e.g.clinic,hospital)mustbedocumentedwithin24hoursoftreatmentusinganInjury/IllnessReport.Theformshouldbecompletedwithasmuchdetailaspossibleandmustincludethehandwrittenemployeestatement(andhandwrittenWitnessStatementifapplicable).Sinceinjuriesthatrequireoff-sitetreatmentaregenerallymoresevereandholdahigherpotentialtobecategorizedas“OSHARecordable”(SeeSection8.6),furtherinvestigationand/orevaluationisrequired.Thedetailsoftheinjuryinvestigation,includingthecorrectiveactions,withownersanddeadlinestoclosethefindings,mustbedocumented(within24hrs).Correctiveactionsshouldbeglobalacrossthesite,applyingtoallsimilarequipmentandallaffectedemployees.Repetitivemotionand/orergonomicrelatedinjuriesmustbereported.AnInjury/IllnessReportmusttobecompleted,dependingontheresultsoftheassessmentand/ortheoptionsavailableforcorrectiveactions.Supervisorsareresponsibleforcompletingandsubmittingassociateddocumentation,however,blankformsareavailabletoallemployeesinbothelectronicandhardcopyformat.CompletedformsmustbesubmittedtotheHRManager.8.3InjuryAssessment&TreatmentAllwork-relatedinjuriesorillnesses,regardlessoftheirseverity,mustfirstbeevaluatedon-sitebyanEMSmember.TheFirstResponderwillassesstheinjuryseverity,providetreatmentand/ordetermineifexternaltreatmentisrequired.Ifanemployeerefusestreatmentforawork-relatedinjury/illness,therefusalmustbedocumentedusingtheTreatmentRefusalCertification,whereanadditionalwitnessmustsigntheform.TheHRManagermustbeimmediatelycontactedforallinjuries/illnessesthatrequireexternaltreatment,orthatareoutsidethescopeoftheEMSmemberstraining.Oncecontacted,theHRMana
