Thepatient,R.S.,wasa54-year-oldAfricanAmericanmalewithhistoryofsevereCOPD;hewasadmittedwithanacuteexacerbationofCOPDandhypoxemicrespiratoryfailure.Onadmission,hewasinacuterespiratorydistress,hisrespiratoryratewas37,pulse136,bloodpressure100/62,andSpO2viapulseoximetrywas82%.Onexamination,R.S.hadanincreasedwork-of-breathingandwasusingaccessorymuscles.Hehaddecreasedairentryonlungexam.HisinitialchestX-rayshowedhyperinflatedlungs.ThepatientwasintubatedonadmissionandtransferredtotheICU.Duringhisstayhewastreatedwithhigh-dosesteroidsforhisCOPDexacerbationandhadtobesedatedandparalyzedinordertomaintainanoxygensaturationabove90%onventilatorysupport.Asthepatientimproved,thesystemiccorticosteroiddosewastaperedandthesedationwasweaned.Earlyinhishospitalcourse,thepatientwasnotacandidateforphysicaltherapyduetohismedicalinstability.Atthepresenttime,thepatient’sbodyisrotatedevery2hbythenurse.Multipodusbootsandhandsplintswereplacedonthepatienttopreventjointcontractures.

ItisnotcompletelyclearwhyweaknessoccursintheICU.Theeffectsofdeconditioningprimarilycomefromstudiesdoneonhealthypersonsplacedatbedrestinspaceprogramsandlow-gravityresearch.Muscleatrophy,lossofforcegeneration,andchangesintypeofmusclefibersalloccurwithbedrest.Ithasbeenfoundthatevenshortperiodsofbedrestaffectskeletalmuscleperformance.After14and35days,muscleforcedecreasesby15and25%,respectively.Thighandcalfmusclevolumesalsodecreasesignificantly.3Whathappenstoskeletalmusclewithdisuseisalsotrueforthediaphragm.Astudythatevaluateddiaphragm-biopsyspecimensfromsubjectsonmechanicalventilationfor18–69hshowedatrophyofbothslowandfast-twitchmusclefibers.4Thisweaknesscontributestoanoveralldecreaseinfunctionalstatusandimpairsventilatoryweaning.

AllpatientsadmittedtotheICUsuffersomeelementofdeconditioningrelatedtotheneedforbedrestandthecatabolicnatureoftheunderlyingdiseasepromptingadmission.ICUpatientswhorequiresedation,neuromuscularblockingagents,corticosteroids,mechanicalventilation,andsufferfromsepsis,shock,and/orrenalfailurerepresentpatientgroupswhoareatgreatestriskfordeconditioning;thesepatientsrequireintensive,multidisciplinarywholebodyrehabilitation.

R.S.hadcontinuedventilator-dependentrespiratoryfailure.Hehadatracheostomytubeplacedforcomfortandmobility,PhysicaltherapywasconsultedintheICU.Thepatientwasassesseddailyforanycontraindicationstophysicaltherapy.Thepatientatthisstagehadpassiverangeofmotionperformedbythenursesthreetimesaday,andthepatient’sbedwasputintoasittingpositionforaminimumof20min3timesaday.ThepatienthadsevereweaknessandCIMfromtheuseofsystemicglucocorticoidsandneuromuscularblockingagentsThepatientprogressedtosittinginachairandrequestedtositforaminimumof45min/day.Thepatientwasunabletoweanfromtheventilatorandwastransferredtotheventilatorrehabunit(VRU)forfurthermanagement.ThepatientmetrespiratoryandnonrespiratorymedicalcriteriaforadmissiontotheVRUtracheostomy,manageablesecretions,stableventilatorsettings,thepresenceofaejunostomyorgast
