基于疾病中医证候分类的中药药效评价研究思路和方法
【摘要】中药的临床应用建立在辨证的基础上,因此其药效评价也应该建立在中医证候分类的基础之上。
通过药效学实验阐明中药治疗疾病的药效作用特点及其机制,揭示中药作用机制的科学内涵,是中药现代化和国际化的必然要求,也是探索药物作用靶点,指导临床合理用药的关键所在。
如何利用成熟疾病动物模型,并明确其证候特征,是中药药效评价载体研究的关键问题。
随着生物医学的发展,通过研究疾病动物模型的生物学指标有可能分析得出该疾病动物模型的证候特性。
利用以方测证方法,建立针对该疾病常见证候类型且临床验证有效的中药干预疾病动物模型,并将治疗有效的动物作为体现相应证候特征的动物模型,通过探寻其现代生物学基础,确立相应的中医证候特征相关指标,从而明确疾病动物模型的证候特征,是建立适合辨证论治中药药效评价动物模型的重要途径。
建立基于中医证候分类的中药药效评价研究方法,将有助于从中医理论出发提高我国中药研发水平。

Thefirstroleofdrugistotreatdiseases.SothepurposeofpharmacodynamicevaluationonChineseherbalmedicineistoclarifythecharacteristicsandactionmechanismsofChineseherbalmedicine.ItisarequestfortraditionalChinesemedicine(TCM)modernizationandakeypartinclinictofocusonpharmacodynamicevaluationonChineseherbalmedicine.Inpresent,alotofresultsonChineseherbpharmacodynamicevaluationarenotsatisfied.Meanwhile,theseresultscannotbeusedtoinstructtheclinicaltrials.ThemainreasonisthattheyignoretheTCMbasictheory,whichiscloselyrelatedtosyndromedifferentiation.TheclinicalapplicationofChineseherbsmustbeestablishedinaccordancewithsyndromedifferentiation, Chineseherbhasitsownindications[1].Hence,thekeyscientificproblemofpharmacodynamicevaluationonChineseherbistoclarifytheChineseherbactionbasedonTCMsyndromedifferentiation.
1SyndromeclassificationandChineseherbpharmacodynamicevaluation
Syndromeclassificationisakindofwaytocarryoutcategorizationfordifferentindividualphysiologyandpathologyfunctionalstatus.ItisthediagnosticmodelandefficacybasisinTCM[2].ChinesemedicinalformulaisthemainforminclinicalapplicationofChineseherbs.Itisthekeyfactortosetupsyndromedifferentiationsystemwhichcomposesspecialformulabasedonsyndromedifferentiation.Theprocessofsyndromedifferentiationistoclearupsyndromeclassificationfirst,andthentoestablishprinciplebasedonsyndrometypeandtodeterminetheformulabasedonprinciplefinally.Thereby,thefinetherapeuticactionofChineseherbcanbedisplayed[3].
Itisacertainrequestforsyndromedifferentiationtobuild“formulasyndrome”correspondence[4].“Formulasyndrome”correspondenceisthepremiseandbasisforinducingChineseherbefficacy.Thehighlightis“selectingcertainformulabasedoncertainsyndrome”[5].IgnoringsyndrometypetoevaluateChineseherbactionsinglehandedlycannotdisplayobjectively.Thustheresultscannotguidetheclinicalpractice.Hence,theclinicalapplicationofChineseherbcloselyrelatestosyndromedifferentiationandthepotencyofChineseherbshouldbeexhibitedunderthespecialsyndrometype.
2AnimalmodelandpharmacodynamicevaluationonChineseherbalmedicine
Thefinalpurposeofselectingdrugistogetfinetherapeuticeffects.ThemainaimforpharmacodynamicevaluationonChineseherbalmedicineistoassessitseffectssystematically.Hence,theanimaldiseasemodelisanimportanttrager[6].Chineseherbsarenormallyusedtotreatonlydefinitepatientswithdefinitesyndrometype.ReseachshowedthattheChineseherbeffectswerenotsatisfactorywithoutthedirectionofsyndrometheory.Andtheresultscouldnotguidetheclinicalpracticebasedonsyndromeclassification.Hence,seekingthesyndromecharacteristicsofanimaldiseasemodelistheimportantcontentforChineseherbpharmacodynamicevaluation.
ItisthekeyproblemforChineseherbpharmacodynamicevaluationtosetupanimaldiseasemodelwithsyndromecharacteristics.Itisdifficulttoembodytheinformationobtainedfrompatientsbyfourdiagnosticmethods,includingsymptoms,tonguepresentation,pulseconditionsandsoon,inananimaldiseasemodel.Soitisnoteasytogetsyndromecharacteristicsoftheanimaldiseasemodelfromtraditionalmethods[7].Followingthedevelopmentofbiomedicine,itispossibletoanalyzethesyndromecharacteristicsofanimaldiseasemodelbybiologicalparameters.Thesyndromecharacteristicsofanimaldiseasemodelcanbeexploredbythemethodof“measuringsyndrometypebymeansofprescription”.UsingclinicallyapprovedeffectiveherbalmedicinetointervenetherelatedanimaldiseasemodelandfindingouttheresponsivenessrelatedbiomedicalparameterswouldbeawaytodefinetheanimaldiseasemodelwithTCMsyndromerelatedtotheherbalmedicine.UtilizingtheanimalmodelwithsyndromecharacteristicstocarryoutChineseherbpharmacodynamicevaluationwillrevealtheadvantageofsyndromedifferentiationanddirecttheclinicalpracticeinselectingsuitablepatientswithcorrectindications.
SomeprogresshasbeenmadeonestablishmentofanimalmodelsforbothdiseaseandTCMsyndrometype,andalsosomepharmacologicalevaluatononsuchanimalmodelsforherbalmedicinehasbeenconducted,howeveritisstillhardtodefinetheTCMsyndromecharacteristics,whichdirectlyinfluencetheguidingroleforclinicaltrialorclinicalpracticesinceTCMsyndrometypeisdifferentiatedonTCMinformationwhichismainlynotshowninanimals.ThussimilarapproachescouldbeusedtofurtherclarifythesyndromecharacteristicsoftheanimalswithcertaindiseaseandTCMsyndrometype.
ThepharmacologicalevaluationofherbalmedicineonanimaldiseasemodelisimportantforTCMmodernization,andalsofornewherbalmedicineresearchandmarketingdevelopmentnationallyandinternationally[810,11].Comparingtheresponsiveandnonresponsiveanimalsafterherbalmedicineinterventionandfindingoutthedifferencesinbiologicalcharacteristicsisascientificwaytoclarifythepharmacologicalmechanismsofherbalmedicineandfurthertoguidetheclinicalpractice.BasedonTCMsyndromeclassification,findingoutthecorrelatedbiomedicalcharacteristicsofresponsiveanimalstreatedwithherbalmedicineisanimportanttaskinpharmacologicalstudyofherbalmedicine.
3ToobtainthebiologicalfeaturesofresponsiveanimalswithdiseaseafterinterventionofherbalmedicineisafeasiblewaytoconductpharmacologicalevaluationofTCMsyndrometype
TCMsyndromeinformationobtainedbyTCMapproachescannotbeobtainedfromanimaldiseasemodel,andTCMsyndromecharacteristicsonanimaldiseasemodelcouldbeonlyshownwithbiologicalparameters.Pharmacologicalevaluationforherbalmedicineonanimaldiseasemodelisalsoshownwithbiologicalchanges.Becausethediagnosticindexesinonediseasearethecommoninallpatients(orallanimaldiseasemodel)withthedisease,theothernondiagnosticbiologicalparametersmightbeawaytosupportthefindingsofbiologicalcharacteristicsofTCMsyndrometype.Theprogressinbasicresearchinlifesciences,suchasprogressinomics,couldbehelpfulinsuchafindingforbiologicalcharacteristicsofTCMsyndrometype.
Thenondiagnosticbiologicalindicatorsarediversifiedandcomplicated,anditisdifficulttosetalimitationforbiologcialparametersforTCMsyndromecharacterizationanditisimpossibletotestallparameters.ThustofindthepotentialbiologicalparametersorpathwaysforTCMsyndromewithsomeappraochescouldbeimportantforTCMsyndromedifferentationinthepharmacologicalevaluation.Followinglargeamountofdataaccumulationinlifesciencesandprogressindataanalysistechnique,itispossibletofindthepotentialsyndromeindexfromdiseaseresearchdata.AccordingtoTCMsyndromeresearchresults,gatheringalltheresearchfindingswhichhavebeenfoundfromtheanimaldiseasemodelandusingtextdataminingtechnologymightbeawaytoexplorethepotentialrelationshipsamongthemodernbiologicalindicatorsforthedisease,andalsotoexplorethesimilarityinbiologicalcharacteristicsbetweendifferentdiseasesiftheysharesameTCMsyndrometypeinsomeway.Thosebiologicalcharacteristicsmightbeincludedforthepharmacologicalevaluationofherbalmedicineonanimaldiseasemodel.Textdataminingistheprocesstogetoutunderstandable,unknown,andultimatelyavailableknowledgefromalargenumberoftextdata[1214].Itcombinesartificialintelligence,machinelearning,naturallanguageprocessing,dataminingandautomatictextprocessingsuchasinformationextraction,informationretrievalandtextclassificationtogether.Textmininghasanimportantvalueinexplorationofpotentialknowledge.Atpresent,theapplicationoftextdataminingtechnologyinthebiologicaldatabasewithhugeamountofdatatofindpotentiallyusefulknowledgehasbecomeafocusofmedicalresearch.Wepreliminarilyinvestigatedthesimilarbiologicalcharacteristicsandpathwaysbetweenrheumatoidarthritis(RA)andulcerativecolitis(UC)withtextminingtechniquesinceRAandUCsharesimilarTCMsyndrometype,andanumberofpotentialcommonbiologicalcharacteristicsandpathwayswerefound.Inaddition,explorationofbiomarkersofTCMsyndrometypeindiseaseswithsystemsbiologyapproaches,suchascombiningwithgenomics,proteomics,metabolomicsandotheromicstechnologiesandmethods,becomesthehotspotsinTCMsyndromeresearch.OurpreviousresultsshowedthatthereweresignificantdifferencesingenomicprofileandmetabolomicalprofilebetweencoldandheatsyndromeinRApatients,whichsuggeststhatomicsmightbehelpfulinclarificationofTCMsyndrome[15,16].
PharmacologicalevaluationforherbalmedicineisoneofthemostimportantresearchareasinTCMmodernizationandnewherbalmedicinediscovery.ThepharmacologicalevaluationforherbalmedicineshouldbebasedonTCMsyndrometype,whichistheonlywaytoshowupthespecificindicationforherbalmedicinebasedonthesyndromedifferentiation,andtheonlywaytotakeTCMadvantagesinherbalmedicineresearchanddevelop
