Wedescribeherethestateoftheartofcertainaspectsconcerningpotentialsmallmoleculetherapydirectedtowardbotulism,byinhibitionofthezinc-proteasecontaininglightchain(LC)ofbotulinumneurotoxinBoNT/AfromtheanaerobicbacillusClostridiumbotulinum.Botulinumneurotoxins(BoNTs)arecomprisedofeightserologically-distinctproteins(A-H),severalofwhicharefurtherdivided,suchasBoNT/Awhichhasfivesubtypes.TheBoNTsarethemosttoxicsubstancesknowntomankind,causingaformofflaccidparalysisthatcanberapidandisoftenlethal.BoNT/Aiscomprisedofa~100kDaheavychain(HC)attachedviaasingledisulfideCys-Cysbondtoa~50kDaLC.TheHCmediatestransporttoanduptakebypresynapticglutamatergicneurons,wheretheLCcleavestheproteinSNAP-25andthuspreventsvesiculartraffickingandreleaseofacetylcholine.TheZn-endoproteaseactivityoftheLCofBoNT/AisatargetforthedevelopmentofsmallmoleculeinhibitorsofBoNT/A-mediatedtoxicity.AvarietyofBoNT/ALCinhibitorshavebeendescribedtodateandwefocushereprimarilyontheZn-binding8-hydroxyquinolinestructuraltypeaswellassomeofthepreviously-describedhydroxamicacids.

BACKGROUND:
Botulismisanacuteformofpoisoningcausedbyoneoffourtypes(A,B,E,F)toxinsproducedbyClostridiumbotulinum,ananaerobic,sporeformingbacillus.Usuallydiagnosisofbotulismisconsideredinpatientswithpredominantmotorsymptoms:
muscleweaknesswithintactsensationandpreservedmentalfunction.
CASEPRESENTATION:
Wereportacaseof56-year-oldCaucasianfemalewithahistoryofarterialhypertension,whopresentedwithacuterespiratoryfailureandbilateralptosismisdiagnosedasbrainstemischemia.Shehadsevereexternalandinternalophtalmoplegia,andautonomicdysfunctionwithneithermotornorsensorysymptomsfromupperandlowerlimbs.Diagnosisofbotulinumtoxinpoisoningwasmadeandconfirmedbyserumantibodytestinginthemouseinoculationtest.
CONCLUSIONS:
Ophtalmoplegia,autonomicdysfunctionandrespiratoryfailurecanbecausedbybotulism.Earlytreatmentandintensivecareisessentialforsurvivalandrecovery.Theelectrophysiologicaltestsarecrucialtocorrectandrapiddiagnosis.Botulism(especiallytypeB)shouldbeconsideredinanycaseofacuteorpredominantisolatedautonomicdysfunction.

forBacillusanthracissporesindairymilkproducts.TheECLassaywassuccessfullyappliedtoscreentypeAClostridiumbotulinumoutbreakstrains.
CONCLUSION:
TheresultsofthestudyindicatethatthisECLassayisverysensitive,rapid(<
6h)andmultiplexinnature.TheECLassayhaspotentialforuseasaninvitroscreeningmethodforBTAsoverothercomparableimmunoassays.
