Abstract
Advancedcommunicationandinformationprocessingtechniquescanbeappliedtounderwaterresearchtoenablenewwaysofenhancingreal-timeexperimentaloutcome.ThispaperpresentssolutionsdevelopedtosupporttheDynamicsoftheAdriaticinRealTime(DART)seatrials,conductedbetweenNURCand26internationalpartnersin2006,whichwerecharacterizedbylargeshore–ship–shoredataexchangerequirements,inexcessof1 GB/day.Thethree-pillarapproachadopted(basedonnetworkinfrastructure,informationinfrastructure,context-baseddatamanagement)involvestheseamlesscombinationofdifferentcommercial-off-theshelfcomponents(369 kb/s2-waysatellitelink,1 Mb/sunidirectionalsatellitelink,molecularsequencereductioncompressionandcaching)andrequiredthedevelopmentofqualityofservicetoolsandpolicies,totranslatescientificneedsfromnaturallanguagetoconfigurationfiles.Resultsshowedthepossibilityoftransferring,overa27-dayperiod,morethan40 GBofdata,meetingexperimentalrequirementswithminimalexpense:
thevariablecostassociatedtothetransmissionof1 MBofdatawasintheorderof10Eurocentsusingatwo-wayVerySmallApertureTerminal(VSAT)systemandof40EurocentsusingDigitalVideoBroadcastingSatellite(DVB-S)unidirectionalservice.AlthoughthesolutionpresentedreferstoaspecificRapidEnvironmentalAssessment(REA)trial,itsapplicationisnotconstrainedtoREAonly.Theflexiblecombinationoftechnologicalcomponentsandwell-provendatamanagementmethodologiesallowsitsapplicationtoabroaderrangeofscientificproblemsassociatedwithnear-real-timeexchangeoflargedatasets.

Themajorroleofdisasterrecoverymanagementistorapidlyrestorethechaoticconditiontonormal,whichincludesmitigation,preparedness,response,andrecovery.Toreducesecondarydamageandloss,therecoveryprocessmustbefacilitated.Informationtechnologies(includingnetwork,web-based,computer,andrelatedsoftwaretools)speeduptheworkprocesstorapidlydeliverdisasterinformationtotherelateddepartment.Inthispaper,weshareanexperiencethatusedaframeworkofemergencyresponse,includingasimplepre-operatingprocessandaweb-basedinformationsystem,toeffectivelymanageandcontroldamagedmedicalequipmentafterafireaccidentinahospitaloperatingroom.Thisweb-basedinformationsystemwasbuiltinonly2 weeksandwasusedtomanagealargeamountofmedicalequipmentaffectedbythefire.Sincethen,iteffectivelypreventedtheinterruptionofclinicalworkat22clinicalunitsbysupportingthereconstructionactivities.

Abstract
Background
Bipolardisordersareamajorpublichealthconcern.Effortstoprovideoptimalcarebygeneralpractitionersandpsychiatristsareunderminedbythecomplexityofthedisorderanddifficultiesinapplyingclinicalpracticeguidelinesandnewresearchfindingstothespectrumofcasesseenindaytodaypractice.
Method
Anationalnetworkofbipolarexpertcentreswasestablished.Eachcentrehasestablishedstronglinkstolocalhealthservicesandprovidessupporttocliniciansindeliveringpersonalizedcareplansderivedfromsystematiccaseassessmentsundertakenatthecentre.
Results
Acommonsetofdiagnosticandclinicalassessmenttoolshasbeenadoptedateightcentres.Evaluationsareundertakenbytrainedassessorsandcross-centrereliabilityismonitored.Awebapplication,e-bipolar©isusedtorecorddatainacommoncomputerizedmedicalfile.Anonymizeddataisenteredintoasharednationaldatabaseforuseinmulti-centreauditandresearch.
Conclusions
Insteadofofferingtreatmentadvicebasedonclinicalpracticeguidelinesrecommendationsforselectedsub-populationsofpatients(a‘top-down’approach),theFrenchbipolarnetworkofferssystematic,comprehensive,longitudinal,andmulti-dimensionalassessmentsofcasesrepresentativeofgeneralbipolarpopulations.This‘bottom-up’strategymayofferamoreefficientandeffectivewaytotransferknowledgeandshareexpertiseasthereferrercanappreciatetherationaleunderpinningsuggestedtreatmentprotocolsandmorereadilyapplysuchprinciplesandapproachestoothercases.Thenetworkalsobuildsaninfrastructureforclinicalcohortandcomparative-effectivenessresearchonmorerepresentativepatientpopulations.
