Theobesityepidemic(肥胖症)isamongthemostcriticalhealthissuesfacingtheUnitedStates.Althoughithasgeneratedalotofattentionandcallsforsolutions,italsohasservedupasuper-sizedportionofmythsandmisunderstandings.
1.Ifyou’reobese,blameyourgenes.
Asobesityrateshavesoared(高涨,猛增),someresearchershavefocusedonindividuals’genetic(遗传的,基因的,起源的)predisposition(倾向,素质,易染病体质)forgainingweight.Yet,between1980and2000,thenumberofAmericanswhoareobesehasdoubled—tooquicklyforgeneticfactorstoberesponsible.
Sowhydoweeatmorethanweneed?
Thesimpleanswer:
Becausewecan.Athomeandatrestaurants,adollarputsmorecaloriesonourplatesthaneverbefore.BeforeWorldWarII,theaveragefamilyspentasmuchas25percentofitstotalincomeonfood—in2011,itwas9.8percent.Andpeopleeatoutnowmorethaninthepast.In1966,theaveragefamilyspent31percentofitsfoodbudgetdiningawayfromhome—in2011,itwas49percent.Becauserestaurantmealsusuallyhavemorecaloriesthanwhatweprepareathome,peoplewhoeatoutmorefrequentlyhavehigherratesofobesitythanthosewhoeatoutless.
Meanwhile,thefoodindustryhasdevelopedtensofthousandsofproductswithmorecaloriesperbite,aswellasnew,effectivemarketingstrategiestoencourageustobuyandconsumemorethannecessary.Weshouldblamethesebusinesspractices,whicharemodifiable(可修饰的,可更改的),forobesityratherthanourgenes,whicharenot.
2.Ifyou’reobese,youlackself-control.
Accordingtoa2006study,“researchonrestrainedeatinghasproventhatinmostcircumstancesdietingisnotafeasible(可行的,可能的)strategy.”Inotherwords:
Peoplewon’tloseweightbytryingtoeatlessbecausetheycan’teasilycontrolthemselves.Unfortunately,thispuritanical(清教徒,极端拘谨的)viewofpersonalresolveplaysdownhowoursurroundingsandmentalstatedeterminewhatweeat.
Researchshowsthatifweareoverwhelmedwithtoomuchinformationorpreoccupied(抢先占有),wehaveatendencytosurrendertopoordietary(规定的食物,食谱)choices.Inonestudy,forexample,peopleaskedtochooseasnackaftermemorizingaseven-digitnumberwere50percentmorelikelytochoosechocolatecakeoverfruitsaladthanthosewhohadtomemorizeatwo-digitnumber.Whenadultsinanotherstudywereaskedtosampleavarietyoffoodsafterwatchingatelevisionshowwithjunk-foodcommercials,theyatemoreandspentalongertimeeatingthanasimilargroupwatchingthesameshowwithoutthejunk-foodads.Inthesamestudy,childrenatemoregoldfishcrackerswhenwatchingjunk-foodcommercialsthanthosewhosawotherads.
Ourworldhasbecomesorichintemptationthatwecanbeledtoconsumetoomuchinwayswecan’tunderstand.Eventhemostvigilant(警惕的,惊醒的)maynotbeuptothetaskofcontrollingtheirimpulses.
3.Lackofaccesstofreshfruitsandvegetablesisresponsiblefortheobesityepidemic.
TheObamaadministration’sHealthyFoodFinancingInitiative(健康食品资助计划)ismeanttohelplow-incomecommunitiesthatlackaccesstofreshfood.AlthoughtheU.SDepartmentofAgricultureestimatesthatfewerthan5percentofAmericansliveinthese“fooddeserts,”about65percentofthenation’spopulationisoverweightorobese.Formostofus,obesityisnotrelatedtoaccesstomorenutritiousfoods,butrathertothechoiceswemakeinconveniencestoresandsupermarketswherejunk-foodmarketingdominates.Sincewearebuyingmorecaloriesthanweneed,eatinghealthfullycouldbemademoreaffordablebyeliminating(消除)unnecessarycheaperlownutrientfoodsandsubstituting(取入,代入)higherqualityfoodsthatmaybeslightlymoreexpensive.
Obesityisusuallytheconsequenceofeatingtoomuchjunkfoodandconsumingportionsthataretoolarge.Peoplemayheadtotheproducesectionoftheirgrocery(食品杂货店)storewiththebestintentions,onlytobeconfrontedbycandyatthecashregisterandchipsandsodaattheendofaisles.Approximately30percentofsupermarketsalesarefromsuchend-of-aisle(通道末端)locations.Foodretailers’impulse-marketingstrategiescontributesignificantlytoobesityacrossthepopulation,notjustforthosewhodonotlivenearagreengrocerorcan’taffordsometimespricierhealthychoices.
4.Theproblemisnotthatweeattoomuch,butthatwearetoosedentary(久坐不动的,坐惯的).
FirstladyMichelleObama’s“Let’sMove”campaignisbasedontheideathatifkidsexercisedmore,childhoodobesityrateswoulddecline.ButaccordingtotheCentersforDiseaseControlandPrevention,therewasnosignificantdecreaseinphysicalactivitylevelsasobesityratesclimbedinthe1980sand1990s.Infact,althoughadropinwork-relatedphysicalactivitymayaccountforupto100fewercaloriesburned,leisurephysicalactivityappearstohaveincreased,andAmericanskeeptipping(倾斜)thescales.
Thereiscompelling(引人注目的)evidencethattheincreaseincaloriesconsumedexplainstheriseinobesity.TheNationalHealthandNutritionExaminationfoundthatpeopletakein,onaverage,morethan500morecaloriesperdaynowthantheydidinthelate1970s,beforeobesityratesaccelerated(加速,促进).That’slikehavingChristmasdinnertwiceaweekormore.Itwouldnotbeaproblemifwestuffed(已经喂饱了的)ourselvesonlyonceayear,butall-you-can-eat(自助餐)feastsarenowavailableallthetime.It’snearlyimpossibleformostofustoexerciseenoughtoburnofftheseexcesscalories.
5.Wecanconquerobesitythroughbettereducationaboutdietandnutrition.
Accordingtoaphysicians’healthstudy,44percentofmaledoctorsareoverweight.AstudybytheUniversityofMarylandSchoolofNursingfoundthat55percentofnursessurveyedwereoverweightorobese.Ifpeoplewhoprovidehealthcarecannotcontroltheirweight,whywouldnutritioneducationalonemakeadifferenceforothers?
Evenwithmoreinformationaboutfood,extra-largeportionsandsophisticated(复杂的,精致的)marketingmessagesundermine(渐渐破坏)ourabilitytolimithowmuchweconsume.ConsiderAmericans’alcoholconsumption:
Onlylicensedestablishmentscansellspiritstopeopleolderthan21,andnoalcoholcanbesoldinvendingmachines(自动贩卖机).YetthereareveryfewstandardsorregulationstoprotectAmericansfromovereating.
Inthe19thcentury,whentherewerenocontrolsonthequalityofdrinkingwater,infectiousdiseasewasamajorcauseofdeath.Oncestandardswereestablished,thenumberofthesefatalities(灾祸,死亡率)plummeted.Similarly,ifAmericansdidnotliveinaworldfilledwithbuffets(自助餐),cheapfastfood,softdrinkswithcornsyrup,andtoomanyfoodswithexcessfat,saltandsugar,theincidenceofobesity,heartdisease,highbloodpressureanddiabetes(糖尿病)probablywouldplummet.Educationcanhelp,butwhat’sreallyneededisregulation—forexample,limitsonmarketingthatcaterstoouraddictiontosugarandfat.

虽然它已经引起了很多的关注也产生了很多的解决方案,但是其中也检验出很大一部分的谬论和误解。
1.如果你是胖子,归咎于你的基因
随着肥胖率猛增,很多研究人员关注一个人的基因对体重增加的影响。
但是在1980年到2000年之间,美国的肥胖人数翻倍,如果说基因是肥胖的决定因素,那这个增速也太快了。
为什么人们吃的比实际需要的要多呢?
答案很简单:
因为人们付得起。
现在,在家里或餐馆,放在自己盘子里的一块钱的热量比以往任何时候都多。
第二次世界大战之前,平均每个家庭在食物上的花费多达总收入的25%-在2011年,占9.8%。
但是人们现在吃了超过以往。
1966年,平均每个家庭在外就餐占饮食预算的31%-在2011年,占49%。
因为餐厅的饭菜通常比我们在家里准备的食物拥有更多的热量,在外吃的次数频繁的人比那些次数少的人更容易变胖。
与此同时,为了让消费者购买比实际需求更多的食品,食品行业把不断推出花样繁多的美食作为有效的市场新战略,而这些产品所含的热量更高。
人们应该怪这些商业运作而不是自身的基因。
2.如果你是胖子,你缺乏自控力
根据一项2006年的研究:
“在多数情况下,节食并不是一个切实可行的减肥方法。
”换句话说,人不会因为努力尽量少吃而减轻体重,因为他们不是那么容易就能控制自己。
新的研究表明,如果人们获得的信息太多或者正专注于某件事,人们更容易选择不健康的食物。
比如,在一项研究中,一组人努力记住一个七位数和一组人记两位数,在记忆任务完成后前一组在巧克力蛋糕与水果沙拉之间选择蛋糕的几率比后一组高50%。
另一项研究中成年人被要求品尝各种零食,收看带有垃圾食品广告电视节目的人比同组那些收看不带食品广告的的吃得更多,且花费的时间更长。
在同一研究中,孩子们看孩子垃圾食品广告比那些看其他广告的吃更多的金鱼饼干。
我们的世界已经充满了诱惑,以至于导致我们花费太多在我们所不能理解的方面。
即使是非常警惕也无法完全控制自己冲动。
3.肥胖者越来越多是因为获取新鲜果蔬的途径少了
奥巴马政府的健康食品资助计划旨在帮助那些无法获得新鲜食品的低收入社区。
虽然据美国农业部门估计,只有不到5%的美国人生活在这些“食物沙漠”,全国约65%的人口为超重或肥胖。
对大多数人而言,肥胖与人们无法获得更有营养的食品无关,而是与人们在垃圾食品占领货架的便利店和超市里所做的购买选择相关。
人们购买的食品所含的热量超过了实际所需,人们要健康饮食就应该减少购买不必要的、营养价值不高的便宜食品,取而代之的是购买价格可能稍贵但是高品质的食品。
肥胖通常是吃了太多垃圾食品且在吃上上面花费巨大的结果。
人们可以前往那些带有良好意图的杂货店,糖果放在在收银台对面,薯条和汽水放在过道的末端。
约30%的超市销售的是在过道末端位置。
食品零售商利用“冲动”而展开的营销策略显著推动了那些住所附近没有绿色杂货商店或无法支付昂贵的健康选择的肥胖人口。
4.问题不是人们吃得太多,而是久坐不动
第一夫人米歇尔·奥巴马的“让我们动起来”活动是基于这样一种想法,如果孩子们增加锻炼,儿童肥胖率将有所下降。
根据美国疾病控制和预防中心的数据,在上世纪80年代和90年代,美国人的体育运动水平并没有显著下降,但是肥胖率却上升了。
事实上,虽然工作有关的体力运动的下降可能减少燃烧多达100的热量,但是休闲体力运动有所增加,这使美国人处于一个平衡值上。
强有力的证据显示,肥胖人数增多是因为人们摄入的卡路里增加了。
美国的全国健康与营养调查显示,人们现在平均每天摄取的卡路里比上世纪70年代末肥胖人数还未猛增时要多500多。
就像吃圣诞大餐,每周两次或更多。
如果我们一年只有一次的喂饱自己,这不会是个问题。
但现在自助盛宴是一直生效的。
所以对于我们大多数人来说燃烧掉这些多余的热量几乎是不可能的。
5.有关饮食与营养的良好教育能帮我们战胜肥胖
根据一项健康调查,44%的男性医生超重。
马里兰大学医护学院的一项研究显示,接受调查的护士中55%超重或肥胖。
如果连提供健康保障的人都不能控制体重,那仅仅靠营养教育有什么用呢?
即使有更多关于食品的信息,超大的分量复杂的营销信息也会逐渐破坏我们限制消费多少的能力。
考虑美国人的饮酒量:
只有持牌机构可以出售烈酒给超过21岁的人,并且在自动售货机里没有酒可以出售。
然而,也有极少数的标准或法规,以保护美国人免受暴饮暴食。
在19世纪,当对饮用水的质量没有监管的时候,感染性疾病是死亡的一个主要原因。
一旦标准被确立,这些死亡人数骤减。
同样,如果人们周围没有充斥着太多的自助餐、廉价快餐、含玉米糖浆的饮料,以及含脂肪、盐和糖过多的食品,美国人患肥胖、心脏病、高血压和糖尿病的几率会大大降低。
教育是有帮助,但人们真正需要的是法律法规,比如限制那些令人容易上瘾的高糖高脂食品的市场准入。
